New Online Enrollment!

The Montebello Unified School District is excited to announce the PowerSchool online student enrollment system for the 2020-2021 school year. This will be for NEW students enrolling in the Montebello Unified School District.  

Our new enrollment system is a secure and streamlined process.  This new system will allow our District to verify all student emergency, medical, and demographic information for the new school year. This new and modern online process will replace many of the hard copy student enrollment forms.

Please gather and prepare the following ORIGINAL documentation:

Verification of Student Date of Birth (at least one of the following)

• Birth Certificate (preferred)
• Passport/Residency Card

Proof of Residence (3 of the following) 

• Required: Utility Bill within last three months (gas, electric, water, home telephone, or cable)
• Required: Driver’s License or ID card with current address
  
  Any one of the following:
   
• Car, Health or Home Insurance Premium
• Medical or other health insurance statement
• Car Registration
• CAL Fresh or Government support program documentation
• Letter from employer on office letterhead
• Check warrant stub
• Credit Card/Bank Statement
• Cellular phone Statement
• Rental agreement that includes name and telephone number of landlord

Parent/Guardian Identification

• Valid government issued photo ID (ie. Driver’s License)

Immunization Records

• Current immunization card
• Physical Exam Report for Transitional Kindergarten, Kindergarten and 1st-grade entry only CLICK BELOW

• Dental Exam for Transitional Kindergarten, Kindergarten and 1st-grade entry only CLICK BELOW

If the following apply:

School Transfer Paperwork:

• Last report card (needed for proper placement).

Individualized Education Program (IEP):

• Required for students receiving special education services.

Section 504 Plan: 

• Required for students receiving accommodations and modifications.

¡Nueva Inscripción en línea! (Solo Estudiantes Nuevos)

El Distrito Escolar Unificado de Montebello se complace en anunciar el sistema de inscripción  de estudiantes en línea a través de PowerSchool para el año escolar 2020-2021. Esto será para estudiantes  NUEVOS que se inscriban en el Distrito Escolar Unificado de Montebello. 

Nuestro nuevo sistema de inscripción es un proceso seguro y simplificado. Este nuevo sistema le permitirá a nuestro distrito verificar toda la información de emergencia, información médica, e información demográfica para el nuevo año escolar.

Este nuevo y moderno proceso en línea reemplazará muchos de los formularios de matriculación en papel.  

Para comenzar a inscribir a su estudiante PRESIONE AQUÍ. 

Reúna y prepare la siguiente documentación ORIGINAL:

Verificación de la fecha de nacimiento del estudiante (al menos uno de los siguientes)

• Acta de nacimiento (preferido)
• Pasaporte/Tarjeta de Residencia

Prueba de residencia (3 de los siguientes) 

• Requerido: Factura de utilidades de entre los últimos tres meses (gas, electricidad, agua, teléfono residencial o cable)
• Requerido: Licencia de conducir o tarjeta de identificación con la dirección actual
  
  Cualquiera de los siguientes:
   
• Prima de auto, salud o seguro de casa
• Declaración de seguro médico u otro tipo de seguro
• Registro de auto
• Documentación del programa de apoyo CAL Fresh o del gobierno
• Carta del empleador con membrete de la oficina
• Talón del cheque de nómina
• Tarjeta de crédito / Reporte de cuenta del banco
• Factura del teléfono celular
• Contrato de alquiler que incluye el nombre y número de teléfono del propietario

Identificación del padre / tutor legal

• Identificación válida con fotografía emitida por el gobierno (ie. Licencia de conducir)

Registros de inmunización

• Tarjeta de vacunación actual
• Informe de examen médico para Kínder Transicional, Kínder, y 1er grado solamente PRESIONE AQUÍ

• Informe de examen dental para Kínder Transicional, Kínder, y 1er grado solamente PRESIONE ABAJO

Si lo siguiente aplica:

Documentación de transferencia escolar :

• Último reporte de calificaciones (necesaria para la colocación adecuada).

Programa de educación individualizada  (IEP):

• Requerido para estudiantes que reciben servicios de educación especial.

Plan de la Sección 504: 

• Requerido para estudiantes que reciben adaptaciones y modificaciones.
